Independent Investigation Ordered for RFNS Puamau Grounding Incident

An independent Board of Inquiry (BOI) will be set up to investigate the recent grounding incident of the RFNS Puamau in Fulaga, Lau.
Republic of Fiji Military Forces Commander Major-General Ro Jone Kalouniwai said the decision to initiate the independent BOI stemmed from the significant monetary value of the affected platform, valued at around $20million, and the recent commissioning of RFNS Puamau into the Fiji Navy (FN) Fleet the last month.
“It is imperative for us to demonstrate our commitment to transparency and accountability by thoroughly examining the mishap in terms of processes and procedures,” Major- General Kalouniwai said last night.

“While acknowledging the internal investigations conducted by the Fiji Navy, the RFMF believes that an independent BOI will enhance the thoroughness and objectivity of the inquiry, ensuring a comprehensive understanding of the incident," he said.
"Our primary goal is to prevent similar occurrences in the future and uphold the highest standards of safety and operational excellence within the Fiji Navy."
Major-General Kalouniwai said the RFMF appreciated the support and cooperation of all stakeholders as they convene the independent investigation process.
He said further updates and information regarding the findings would be disseminated in due course.
Meanwhile, RFMF Naval Division Commander, Commodore Humphrey Tawake said a salvage operation vessel would depart Suva today to start its salvage operation on RFNS Puamau.
Commodore Tawake confirmed the vessel would be arriving at the site by Wednesday.
"Our current action now is we're removing all the fuel by tomorrow midday," he said.
"Given the bad weather, we're being cautious about this operation."
Commodore Tawake said crew members of RFNS Savenaca and six divers were currently in Fulaga with a team from Australia.
"There are only two compartments that's underwater, the other three are above water," Commodore Tawake said.
"The central compartment is under water and we're trying to control it. So far so good, and we're trying our best.
"The team at Fulaga is just waiting for the salvage experts to arrive there."
