A preliminary report into the death of timber machine operator Ravinesh Prasad has found signs of negligence by both the employer and the worker.

Minister for Employment, Productivity and Workplace Relations Agni Deo Singh confirmed the findings while responding to media queries in Labasa yesterday.

It is alleged the 47-year-old was working near an industrial machine on March 6 when he became entangled in the equipment.

He sustained serious head and rib injuries and was unable to communicate with his family while receiving treatment in hospital.

Mr Prasad died at Labasa Hospital at about 3:08am on March 9. He is survived by his wife, Shareen Devi, and three sons aged 25, 19 and 14.

Mr Singh described the incident as an unfortunate loss for the family, noting Mr Prasad had joined Pinto Industries Pte Limited in January this year.

He confirmed the preliminary report had been received and further action was being considered.

“The next step in terms of taking action is being worked on by the senior staff at the ministry,” he said.

Mr Singh said the case raised serious Occupational Health and Safety concerns.

“We are definitely addressing this OHS non-compliance issue,” he said.

“I have not yet been fully advised on the report itself, it is being handled by the deputy secretary health and safety, once it comes to my table, I will be able to identify in full detail whether the employer is entirely at fault or not”

“So far, the preliminary report shows that there was some negligence by the employer, in terms of making sure that preventative measures are taken”

“At the same time, I believe the worker was not being as careful as he should have been,” he said.

Following the incident, Pinto Industries director Vishal Narayan said the company was compliant with all OHS regulations.

Police spokesperson Sergeant Wame Bautolu confirmed investigations are ongoing.
